Zusammenfassung
Bei Patienten mit Diabetes mellitus und/oder chronischem Glaukom tritt nach ophthalmologischen Eingriffen gehäuft eine Fibrinreaktion auf. Nach Organisation von Fibrin kann zur Auflösung eine intraokulare Applikation von Fibrinolytika erwogen werden. Der „rekombinante Tissue Plasminogen Activator” (r-tPA) führt zur Lyse von intraokularen Fibrinmembranen. 13 Patienten mit massiver postoperativer Fibrinreaktion wurden mit r-tPA postoperativ behandelt. Die gestörte Blut-Kammerwasser-Schranke mit Fibrinbildung war durch mehrmalige Voroperationen und die Anamnese erklärt. Einmalig wurden 0,1 ml mit 20 μg r-tPA Lösung in die Vorderkammer injiziert. Frühester Injektionszeitpunkt war der 4. postoperative Tag. Bei allen mit r-tPA behandelten Patienten war am 1. Tag post injektionem kein Fibrin mehr nachweisbar. Bei 2 Patienten trat am 2. Tag nach r-tPA-Injektion eine erneute Fibrinreaktion auf. Komplikationen wie z.B. Druckanstieg, Blutungen und Entzündungen wurden nicht beobachtet. Die aufgezeigten Ergebnisse belegen, daß eine intraokulare r-tPA-Injektion zur Auflösung von Fibrin und Vermeidung sekundärer Komplikationen geeignet ist.
Summary
In patients with diabetes mellitus and/or chronic glaucoma fibrin reactions are frequently seen after surgical intervention. In case of formation of a fibrin membrane intraocular injection of fibrinolytics should be considered. Tissue plasminogen activator can dissolve ocular fibrin membranes. 13 patients with marked fibrin reaction were treated with r-tPA post surgery. The impaired barrier of blood and aequeous fluid, the reason for fibrin development, is explained by repeated operations and preexisting systemic diseases. Only a single injection of 0.1 ml of 20 mg r-tPA solution was given into the anterior chamber. The earliest time of injection was the fourth day after surgery. No fibrin was provable in all with r-tPA treated patients on the first day after injection. Two patients got again a fibrin reaction on the second day after injection. Complications like increasing tension, bleeding or inflammation were not noticed. Our results point out that intraocular r-tPA injection is suitable for solving fibrin and avoiding further complications.
