Zusammenfassung
Von 1989 bis 1992 wurden an unserer Klinik 15 Augen von insgesamt 12 Kindern primär oder sekundär mit einer Intraokularlinse (IOL) versorgt. Das Alter lag bei 7 Kindern zwischen 3 und 7 Jahren, bei 5 Kindern zwischen 11 und 15 Jahren. Ursache der Aphakie war bei 6 Kindern ein monokulares Trauma mit primärer Aphakie und sekundärer Katarakt. 6 Kinder wiesen eine juvenile Katarakt auf, 3 davon beidseitig. Primär implantiert wurden nach extrakapsulärer Kataraktextraktion insgesamt 10 Linsen, davon 9mal eine HKL und einmal eine VKL. Sekundär implantiert wurden 5 Linsen, davon einmal eine HKL und 4mal eine VKL. Intraoperativ wurde 2mal eine vordere Vitrektomie und 3mal eine gleichzeitige Nachstarexzision durchgeführt. Postoperative Komplikationen waren bei 5 Augen fibrinöse Exsudationen in der Vorderkammer und bei 2 Augen Pigmentbeschläge auf der IOL. Bei einseitig traumatischer Aphakie konnte bei 2 Augen der beste Visus von 0,9 bis 1,0 erzielt werden, bei den übrigen 4 Augen betrug er 0,2 bis 0,6. Bei beidseitiger juveniler Katarakt und primärer Implantation erreichten alle 6 Kinder einen Visus von 0,8. Kinder mit einer einseitigen juvenilen Katarakt hatten nach primärer oder sekundärer IOL-Implanta-tion einen maximalen Visus von 0,6. Wir empfehlen daher, bei Kindern ab dem 3. Lebensjahr primär und sekundär eine Linse zu implantieren, da das gute visuelle Endergebnis die geringe intra- und postoperative Komplikationsrate aufhebt.
Summary
Between 1989 and 1992 at the Eye Clinic of Bremen in 15 eyes from a total of 12 children a primary or secondary IOL has been implanted. 7 Children aged between 3 and 7 years, 5 between 11 and 15 years. 6 Children had a monocular trauma with primary aphakia or secondary cataract. 6 children had a juvenile cataract, 3 of them in both eyes. As a primary procedure after extracapsular cataract extraction 10 IOL’s have been implanted (9x PCL and 1 xACL). As a secondary procedure 5 lenses have been implanted (1 xPCL and 4xACL). Intraoperatively in 2 eyes a vitrectomy has to be performed and in 3 eyes a simultaneous excision of secondary cataract formation. Postoperatively in 5 eyes fibrineous exsudates have been seen in the anterior chamber and in 2 eyes pigment deposits on the IOL. Concerning the visual acuity in 2 eyes with monocular traumatic aphakia the best visual result of 0.9 to 1.0 could be achieved, in the other 4 eyes between 0.2 and 0.6. In case of juvenile cataract in both eyes and primary implantation all 6 children had a visual result of 0.8. Children with monocular juvenile cataract after primary or secondary IOL implantation had the best visual acuity of 0.6. Out of our experience we therefore recommend the primary and secondary implantation of an IOL in children over 3 years old because of the low rate of intra- and postoperative complications and the really good visual result.
