Scoring systems and computer-aided prognosis in patients with bleeding oesophageal or gastric varices

Summary

In patients with bleeding oesophageal or gastric varices, the Child scoring system consisting of 5 variables provides a popular model for prognosis. We performed a study on the adequacy of this expert scoring system by using a prospective clinical data set (n=82) and statistical methods to provide scores (logistic regression, independence Bayes). By logistic regression it could be demonstrated that the dimensionality of the Child scoring system could not be reduced. Rough agreement between the Child scores and the statistical scores was observed for the variables ascites, encephalopathy and prothrombin index. Moderate differences occurred for albumin and bilirubin. It is concluded that statistical scoring does not markedly improve the definition of risk groups.

Zusammenfassung

Bei Patienten mit blutenden Ösophagus- oder Fundusvarizen wird das Child-Punktesystem bestehend aus 5 Parametern häufig zur Beschreibung der Prognose verwandt. Auf der Basis eines prospektiven klinischen Datensatzes (n=82) und mit statistischen Methoden zur Bildung von Scores (logistische Regression, Unabhängigkeits-Bayes) wurde die Adäquatheit dieses Expertenscores untersucht. Durch logistische Regression konnte nachgewiesen werden, daß eine Reduzierung des Punktesystems auf weniger Variablen nicht sinnvoll ist. Weitgehende Obereinstimmung der Punktebewertung im Child-Punktesystem mit den statistisch abgeleiteten Scores konnte für die Variablen Aszites, Enzephalopathie und Quick beobachtet werden. Unterschiede wurden dagegen bei den Variablen Albumin und Bilirubin festgestellt. Es wird gefolgert, daß die Bildung von statistischen Scores die Definition von Risikogruppen nicht entscheidend verbessert.
