Zusammenfassung

Die Chirurgie ist in frühen Stadien des Magenkarzinoms Therapie der Wahl. In fortgeschritteneren, irresektablen und metastasierten Stadien ist eine systemische Chemotherapie (CTx) indiziert. In neueren Studien induzierte die perioperative CTx bei technisch inoperablen Tumoren eine Verlängerung der medianen Überlebenszeit im Vergleich zur alleinigen Chirurgie und bei einem Teil der Tumoren (47–80%) konnte nach CTx reseziert werden. Jüngere Patienten mit diesen Tumorstadien sollten deshalb innerhalb kontrollierter Studien mit effektiver Chemotherapie wie z.B. EAP behandelt werden, um bei objektiver Remission eine Resektion verbliebener Resttumoren durchzuführen.

Summary

Surgery is the treatment of choice in early stages of gastric carcinoma. Systemic chemotherapy (CTx) is indicated in unresectable and metastatic disease. New studies have shown that CTx perioperatively improved median survival times compared with surgery alone in locally advanced, technically inoperable tumors. Moreover CTx induced resectability in 47–80% of the patients. Therefore, younger patients in these stages of disease should be treated with effective CTx, such as EAP, in order to induce objective remissions so that residual tumor masses can be surgically removed.
