Summary

Lymphadenectomy, including the lymphatic drainage paths N₂ in gastric cancer as well as in colon cancer, is a precondition for adequate tumor staging. From the surgical point of view lymphadenectomy leads to an increase in the ratio of resections and the rate of R₀ resections (complete tumor resection). So far a reduction of the rate of local recurrencies has been proven only for colorectal carcinomas. Lymphadenectomy seems to improve prognosis in patients with N₁ metastasis. Despite extension of the surgical procedure lymphadenectomy does not result in increased mortality but a slight increase in morbidity can not be excluded.

Zusammenfassung

Eine Lymphadenektomie bis incl. der Lymphabflußwege N₂ ist sowohl beim Magen-wie auch beim Coloncarcinom Voraussetzung für ein adäquates Staging des Tumors. Aus chirurgischer Sicht führt die Lymphadenektomie zu einer Erhöhung der Resektionsquote und zu einer Steigerung der Rate von R₀-Resektionen (komplette Tumorentfernung). Eine Reduktion der Rate an Lokalrezidiven ist bislang nur für das colorectale Carcinom belegbar. Zu einer Prognoseverbesserung scheint die Lymphadenektomie bei Patienten mit einer N₁–Metastasierung zu führen. Trotz der Erweiterung des chirurgischen Eingriffes durch die Lymphadenektomie kommt es zu keiner Steigerung der Letalität, eine geringfügige Zunahme der Morbidität ist aber nicht auszuschließen.
