Mehrmalige erfolgreiche operative Resektion eines Kardia-Karzinom-Rezidivs

Summary

The authors report on a 76-year-old patient with cardia carcinoma and partial gastric resection as well as oesophagogastric anastomosis in 9/96 from outside our hospital (Histo: pT3, pN2, Rx, G4). In 11/96, there was anastomotic adeno carcinoma recurrence, including the distal oesophagus. Surgical treatment: resection of the anastomosis, residual gastric resection as well as end-to-side oesophagojejunal anastomosis (Roux-Y) outside our hospital (Histo: pT3, pN2, R1, G4). In 1/97, anastomotic adeno carcinoma recurrence occurred again. Good condition of the patient and no secondary dissemination of the carcinoma. Surgical treatment: transhiatal oesophagectomy with jejunal interposition and cervical anastomosis in our hospital (Histo: pT4, pN2, R0, G4). 9 months of carcinoma-free life.

Zusammenfassung

Berichtet wird über eine 76-jährige Patientin mit Magen-Kardia-Karzinom und oberer Magenteilresektion sowie Restmagen-Oesophagostomose 9′96 in einem auswärtigen Haus. Histo: pT3, pN2, Rx, G4. Bei Adeno-Karzinomrezidiv im Anastomosenbereich mit Beteiligung des distalen Oesophagus 11′96 Anastomosenresektion, Restmagen-Nachresektion und Blindverschluß sowie termino-laterale Oesophago-Jejunostomose nach Roux-Y. Histo: pT3, pN2, R1, G4, Lymphangiosis carcinomatosa. Bei erneutem Rezidiv im Anastomosenbereich, jedoch gutem Allgemeinzustand und ohne sekundäre Tumormanifestation 1′97 in unserem Haus transmediastinale Oesophagusresektion mit Jejunuminterponat und collarer Anastomose. Histo: pT4, pN2, RO, G4. Neun Monate tumorfreies Überleben.
