Summary
Pancreatic pseudocysts have only been reported in four cases in the fetus or newborn. We report another case of a histologically proven pancreatic pseudocyst in a neonate, which was detected intraoperatively. A prenatal sonogram had suggested the presence of a cystic abdominal mass, which was confirmed postnatally. An upper GI examination showed a duodenal sweep to be compressed by an extraluminal mass. Laparotomy was performed in the second week of life, because of the increase in lipase, amylase and bilirubin, although the cystic mass was getting larger. Intraoperatively a cystic mass in the head of the pancreas was found. Because of the location of the cyst, internal drainage was preferred. In contrast, those cysts located within the pancreatic body and tail could be surgically excised. Congenital pancreatic pseudocysts are extremely rare; however, this entity should be given consideration when cystic lesions are detected in the upper abdomen during the fetal and newborn periods.
Zusammenfassung
Kongenitale Pankreaspseudozysten wurden bisher in der Weltliteratur nur in 4 Fällen veröffentlicht. Wir stellen einen Fall einer histologisch gesicherten Pankreaspseudozyste beim Neugeborenen vor, die erst intraoperativ zugeordnet werden konnte. Bereits präpartal wurde sonographisch eine zystische Raumforderung im Oberbauch beschrieben, was sich postpartal bestätigte. In der MD-Passage zeigte sich, daß das Duodenum durch eine extraluminäre Raumforderung bogig aufgespannt war. In der 2. Lebenswoche wurde eine explorative Laparotomie durchgeführt, da es zu einem starken Anstieg von Lipase, Amylase und Bilirubin kam. Wegen der Lokalisation im Pankreaskopf wurde eine Zystojejunostomie durchgeführt. Wäre die Zyste im Korpus- oder Schwanzbereich gelegen, so könnte eine Resektion erfolgen. Eine kongenitale Pankreaspseudozyste ist sehr selten, dennoch sollte sie in die Differentialdiagnose einer zystischen Struktur im Oberbauch mit eingeschlossen werden.
