Epidemiologie von Alkoholmißbrauch und -abhängigkeit

Zusammenfassung

In Ländern mit hohem Alkoholkonsum ist der daraus resultierende wirtschaftliche Schaden erheblich. Epidemiologische Untersuchungen können dazu beitragen, Hochrisikogruppen zu identifizieren, Ursachen für die Entstehung von Alkoholismus zu finden und empirisch fundierte Präventionsmaßnahmen zu entwickeln. In zwei Studien, der Oberbayerischen Verlaufsuntersuchung und der amerikanischen Epidemiological Catchment Area Study, wurden jeweils über 1800 Probanden bevölkerungsbezogener Stichproben hinsichtlich ausgewählter psychischer Erkrankungen untersucht. In beiden Studien war Alkoholmißbrauch und -abhängigkeit die diagnostische Kategorie mit der höchsten Prävalenz (2,9 % in den USA bzw. 3,1 % in Oberbayern). Alkoholmißbrauch und -abhängigkeit waren unter Männern jeweils um ein Vielfaches häufiger als bei Frauen. In der oberbayerischen Studie war zudem Alkoholismus in niedrigen sozialen Schichten deutlich mehr als in höheren sozialen Schichten verbreitet. Eine andere amerikanische Studie (NCS) ergab eine besonders hohe Assoziation zwischen Alkoholmißbrauch und -abhängigkeit einerseits und Manie und Schizophrenie andererseits. Das Vorkommen einer weiteren psychischen Erkrankung war bei Personen mit Alkoholismus mit einer ungünstigen Remissionsrate verbunden.
