Intraarterial Digital Subtraction Angiography for Postoperative Determination of Aortocoronary Bypass Graft Patency

Abstract

Efforts have been made in recent years to establish a postoperative control method to determine the patency of aortocoronary bypass grafts by the development of noninvasive procedures. According to the literature, graft occlusion is expected to occur in 8%–20% of grafts [3, 4, 10] during the first days or weeks after operation. In about another 10% low to middle grade stenoses are visualized at the proximal and distal anastomoses without any clinical symptoms. None of these noninvasive procedures such as directional Doppler sonography [15], myocardial scintigraphy [11], and angio-sequence CT [2, 7, 9, 14, 17] are able to visualize the graft through its whole course. The quality of the distal anastomoses, which is decisive for the prognosis of the bypass, cannot be judged. Even with transvenous digital coronary angiography, demonstration of the distal anastomoses is not satisfactorily possible [6].
