Zusammenfassung
Eine parenterale Nährstoffzufuhr durch intravenöse Infusionen wird notwendig, wenn eine bedarfsgerechte Nährstoffzufuhr auf oralem oder enteralem Wege über einen längeren Zeitraum nicht realisiert werden kann. Bevor eine parenterale Ernährung begonnen wird, sollte gründlich geprüft werden, ob nicht eine ausreichende enterale Nährstoffzufuhr erreichbar ist. Die intravenöse Nährstoffzufuhr kann als ergänzende oder partielle parenterale Ernährung eine zwar vorhandene, aber insgesamt unzureichende enterale Nährstoffzufuhr ergänzen, wie es beispielsweise während der Phase eines allmählichen enteralen Nahrungsaufbaus bei einem Frühgeborenen der Fall ist. Dagegen kann die totale parenterale Ernährung eine ausschließliche oder ganz überwiegend intravenöse Nährstoffzufuhr gewährleisten, z. B. nach operativen Eingriffen im Abdominalraum oder bei akuten bzw. chronischen Darmkrankheiten mit gestörter Nahrungstoleranz oder schwerer Malassimilation. Allerdings soll neben der totalen parenteralen Ernährung soweit wie möglich auch eine zumindest minimale enterale Nahrungszufuhr angestrebt werden. Auch wenn eine solche ergänzende minimale enterale Ernährung keinen nennenswerten Beitrag zur Deckung des Substratbedarfs beiträgt, kann sie das Risiko potenzieller Komplikationen wie einer Mukosaatrophie oder der Entwicklung einer Cholestase vermindern.
