Zusammenfassung

Bei der Chirurgie des Rektumkarzinoms auf dem Boden einer chronisch entzündlichen Darmerkrankung treffen drei etablierte operative Strategien mit unterschiedlichen Ansätzen aufeinander: Die multimodale Therapie des Rektumkarzinoms mit chirurgisch-onkologischen Prinzipien und (neo-) adjuvanten Therapiekonzepten, die spezifische darmsparende Chirurgie beim Morbus Crohn und der restaurative Ansatz mit Ileumpouchanlage bei der Colitis ulcerosa. Die Herausforderung besteht in der Wahl der optimalen Strategie unter Wahrung der onkologischen Prinzipien und Berücksichtigung der Ansprüche und Möglichkeiten in Bezug auf die Lebensqualität. In wieweit sich die verschiedenen Ansätze miteinander vereinen lassen, soll im folgenden Kapitel anhand der retrospektiven Datenlage dargestellt werden.
