Zusammenfassung

Die klassische Indikation zur simultanen Pankreas-/Nierentransplantation stellt der juvenile Typ-1-Diabetiker mit negativem C-Peptid und (prä-)terminaler Niereninsuffizienz dar. Nichturämische Typ-1-Diabetiker mit schweren, rezidivierenden Stoffwechselentgleisungen, Vorhandensein von Hypoglykämiewahrnehmungs- und Gegenregulationsstörungen sowie einem anhaltenden Versagen einer intensivierten Insulintherapie sollten für eine alleinige Pankreastransplantation evaluiert werden. Bei Typ-1-Diabetikern nach erfolgter Nierentransplantation mit stabiler Funktion kann eine Pankreastransplantation zu einer langfristigen Normoglykämie führen. Die Pankreastransplantation bei Typ-2-Diabetikern sollte nach kritischer Empfängerselektion nur in Ausnahmefällen erfolgen. In der Selektion von Spendern und Empfängern macht sich die demographische Entwicklung der Bevölkerung deutlich bemerkbar.
