Zusammenfassung
Die intraduktale Druckerhöhung mit Pankreasgangstenosen und Pankreasgangsteinen als Korrelat der Abflussstörung kann zur Schmerzgenese und Auslösung von Schmerzattacken bei der chronischen Pankreatitis beitragen. Eine Entlastung des Pankreasganges unter Einbeziehung von Pankreasgängen II. und III. Ordnung kann hierbei gute Ergebnisse mit Schmerzfreiheit liefern. Während die klassischen reinen Drainageoperationen mit laterolateraler Pankreatikojejunostomie im Langzeitverlauf in der Mehrzahl der Fälle nicht zu einer Schmerzfreiheit führen, liefern die Hybridverfahren mit Erweiterung der Drainageoperation auf den Pankreaskopf vergleichbare Ergebnisse zu den resezierenden Verfahren. Sollte eine Rekanalisation der Pfortader nicht möglich sein oder ein fixierter und langstreckiger Pfortaderverschluss vorliegen, so findet sich in dieser Indikation eine gute Einsatzmöglichkeit für die drainierenden oder besser noch die erweitert drainierenden Verfahren.
