Zusammenfassung
Der Gebrauch legaler und illegaler Rauschmittel führt zu erheblichen Schäden beim Konsumenten selbst und in der Gesellschaft; dabei führt zahlenmäßig Alkohol vor Heroin, Crack-Kokain und den Designerdrogen. Besprochen werden die Maßnahmen zur Sicherung der Vitalfunktionen, die Entgiftung und ggf. Sedierung in den Intoxikations- und Entzugssituationen. Zusätzlich legt dieses Kapitel die klinischen und diagnostischen Probleme des oft verschwiegenen Konsums sowie die wichtigen Drogenkomplikationen dar. Im zerebralen Allgemeinsyndrom dieser Gruppe von Enzephalopathien sind Tremor, Krampfanfälle und vegetative Symptome häufig anzutreffen. Sie und zusätzliche fokale Symptome werden jedoch auch durch Komplikationen des Abusus wie Sepsis, ZNS-Trauma nach Sturz und Schlaganfall ausgelöst, sodass auch in Anbetracht der Häufigkeit des Substanzmissbrauchs eine Ausschlussdiagnostik meistens unumgänglich ist. Leider ist die Mischintoxikation mit entsprechend verzerrter Symptomatik häufig und ebenso einzukalkulieren wie eine unklare anamnestische Datenlage (Cave: Body-packer-Syndrom).
