Zusammenfassung

Maligne Tumorerkrankungen in einem metastasierten Stadium wurden bisher in palliativer Intention behandelt. Neueste Fortschritte im Bereich der Bildgebung, der perioperativen Behandlung, der chirurgischen und interventionell regionalen Therapie sowie die Einführung wirksamer Chemotherapieprotokolle haben zu einer signifikanten Prognoseverbesserung für diese Patientengruppen geführt [1]. Dabei sind Faktoren wie die Tumorgröße, Anzahl der Metastasen sowie vorhandene Restleber entscheidende Parameter für eine entsprechende Indikationsstellung [2]. Der Fokus ist dabei gerichtet auf die noch funktionierende Restleber. Unabhängig von der zugrunde liegenden Histologie muss bei der Therapieentscheidung bei Lebermetastasen prinzipiell zwischen palliativer Therapie, symptomatischer Therapie und neoadjuvanter Therapieindikationsstellung bis hin zur kurativen Therapie unterschieden werden. In der Regel sind heute insbesondere beim kolorektalen Karzinom die Therapiekonzepte der Behandlung von Metastasen multimodal.
