Zusammenfassung

Mädchen in der Pubertät und Adoleszenz und junge erwachsene Frauen stellen eine Risikogruppe für die Entwicklung einer Essstörung dar. Trotz einer uneinheitlichen Studienlage kann davon ausgegangen werden, dass ein Typ-1-Diabetes in diesen Altersgruppen ein zusätzliches Risiko darstellt, zumindest an einer subklinischen Essstörung (nicht näher bezeichnete Essstörung) zu erkranken. Die Komorbidität von Typ-1-Diabetes und einer (subklinischen) Essstörung geht mit früh auftretenden und ausgeprägten diabetischen Spätfolgen einher. Insbesondere die bewusste Reduktion von Insulin als gegenregulatorische Maßnahme gegenüber einer Gewichtszunahme (z. B. nach einem Kontrollverlust bei der Nahrungsaufnahme), das sogenannte „Insulin-Purging“ stellt nicht nur einen ernsten Risikofaktor für die Entwicklung diabetischer Spätschäden dar, sondern geht auch mit einer höheren Mortalität einher. Der Typ-2-Diabetes ist häufig mit Übergewicht und Adipositas assoziiert. Im Rahmen der multifaktoriellen Genese von Übergewicht und Adipositas kommt der Binge-Eating-Störung eine differenzialdiagnostische Bedeutung zu. Die Binge-Eating- Störung scheint bei Patienten mit Typ-2-Diabetes nicht häufiger aufzutreten als bei stoffwechselgesunden Menschen. Ihre ätiologische Bedeutung für Übergewicht und Adipositas und konsekutive Insulinresistenz ist jedoch zu berücksichtigen und entsprechende therapeutische Implikationen zu beachten.
