Zusammenfassung
Rückenschmerzen zählen in Deutschland und anderen Industrienationen zu den größten Gesundheitsproblemen. Sie bedingen eine erhebliche individuelle Beschwerdelast sowie hohe Kosten im Gesundheitssystem und in der Volkswirtschaft. Betroffen sind im Laufe eines Jahres rund 55–75 % der Erwachsenenbevölkerung. Der Begriff Rückenschmerz beschreibt zahlreiche Formen schmerzhafter Beschwerden im Rücken, denen nur minderheitlich medizinisch eindeutige Ursachen zugeordnet werden können. Insbesondere bildgebende Verfahren haben nur eine begrenzte diagnostische und therapieleitende Aussagekraft. Unter den sog. spezifischen Ursachen dominieren degenerative Erkrankungen. Die Mehrheit der akuten unspezifischen Rückenschmerzen klingt innerhalb weniger Wochen ab. Wichtige Risikofaktoren für die Chronifizierung sind psychosoziale Faktoren wie Depressivität oder Stress. Während Rückenschmerzen in allen gesellschaftlichen Schichten vorkommen, sind beeinträchtigende Rückenschmerzen in sozial schwachen Schichten stark überrepräsentiert.
