Zusammenfassung
Einführung
Unter dem Begriff interventionelle Radiologie werden minimalinvasive Eingriffe zusammengefasst, die bildgebungsgesteuert durchgeführt werden. Mit der klinischen Einführung der Röntgenfluoroskopie und der Angiographietechniken, sowie später der Schnittbildgebung (Ultraschall, Computertomographie, Magnetresonanztomographie) wurde nicht nur das diagnostische Potenzial genutzt, sondern simultan auch die Anwendung dieser Technologien zur Führung von Instrumenten vorangetrieben. Die Vorteile interventioneller Techniken liegen in der minimalen Invasivität und des lokal begrenzten Traumas begründet. Das Gewebstrauma zur Exposition des Interventionsziels bei offener Chirurgie entfällt ebenso wie das allgemeine operative und anästhesiologische Risiko, ein wichtiger Aspekt v. a. bei polymorbiden Patienten, bei erhöhtem kardiovaskulärem Risiko und in der kritischen postoperativen Phase.
Komplexe Interventionen (sog. Hybridverfahren) werden unter kombiniertem Einsatz mehrerer Bildgebungstechniken durchgeführt. Die Eingriffe werden in der Regel in Lokalanästhesie und Analgosedierung durchgeführt. Die Patientenvorbereitung orientiert sich an den Richtlinien operativer Standards (Nahrungskarenz, Laborcheck, ggf. Antibiotikaprophylaxe etc.) Aus früheren Angiographiearbeitsplätzen sind inzwischen Interventionsoperationssäle geworden, die den Hygienestandards und Organisationsabläufen in operativen Einheiten angelehnt sind.
