Zusammenfassung
Grundsätzlich gibt es zwei Möglichkeiten, den Säure-Basen-Haushalt (SBH) zu interpretieren: nach dem konventionellen Ansatz, so wie er nachfolgend dargestellt werden soll, und nach dem sog. physikochemischen Ansatz, wie er 1981 von Peter A. Stewart vorgeschlagen wurde [23]. Der Stewart-Ansatz zeigt schlüssig, dass Bikarbonat- und Protonenkonzentrationen abhängige Variable sind, da sie letztlich von Veränderungen anderer in Lösung befindlicher Ionen abhängen. Eine deutschsprachige Übersicht dazu findet sich bei Rehm [19], Deetjen u. Lichtwarck-Aschoff [8] sowie umfassend bei Kellum u. Elbers [14] in englischer Sprache. Für die Interpretation des Säure-Basen-Haushalts mit dem Stewart-Ansatz ist jedoch u. a. die Bestimmung der Plasmakonzentrationen von Albumin, Phosphat, Chlorid, Magnesium und Kalzium erforderlich: Messwerte, die die derzeitig auf dem Markt befindlichen Blutgasanalysatoren nicht liefern.
