Abstract
Small airways disease (SAD) is defined as a pathological condition in which the small conducting airways are affected either primarily or in addition to alveolar or interstitial lung changes. Diseases affecting the small airways are often difficult to detect and to quantify by traditional diagnostic tests. The pulmonary function deterioration is indeed not only the result of small airways involvement but is also related to associated lung parenchyma changes or large airways involvement that are often present. Many histopathological subtypes have been defined. A problem is that these subtypes do not always have obvious clinical correlates, and even when there are obvious correlates, confusion can be induced by the fact that limited disease shown on pathological or radiological examinations does not necessarily result in symptoms or functional deterioration that needs to be present to make the clinical diagnosis. The increased use of high resolution computed tomography (HRCT) to study the appearances of the various pathologic subtypes is responsible for an enormous progress in the understanding of small airways disease.
