Zusammenfassung

Die Einteilung der Gewebetransferverfahren in gestielte und freie Transplantationen spiegelt die Geschichte der plastischen Deckung von tumorbedingten oder traumatisch verursachten Defekten wider und ist durch die Fortschritte des anatomischen Grundlagenwissens und der instrumentell-technischen Möglichkeiten begründet. Die erfolgreiche klinische Realisierung der freien mikrochirurgischen Gewebetransplantation zu Anfang der 1970er Jahre gab dabei Anlass dazu, die Unterscheidung in gestielte und freie Lappenplastiken als ein Hauptkriterium zur Klassifikation von Lappentransplantaten festzuschreiben (Antia u. Buch 1971, Taylor u. Daniel 1973). Konträr zu den freien Lappenplastiken (»free flaps«), bei denen die nutritiven Gefäße freipräpariert, getrennt und in einer entfernt liegenden Empfangsregion reanastomosiert werden, bleibt bei den gestielten Lappen (»pedicled flaps«) die Vaskularisation über eine Gewebebrücke (Haut, Subkutis) oder über einen isolierten Gefäßstiel während der Einheilungsphase aufrechterhalten.
