Zusammenfassung
Infektiöse Erkrankungen des Herzmuskels können in jedem Lebensalter auftreten. Ursächlich als Erreger kommen v. a. verschiedene Virusarten, am häufigsten Coxsackie-Virus, in Betracht. Oftmals führt die systemische Erkrankung mit diesen Infektionserregern auch zu einer Mitbeteiligung des Herzmuskels in Form einer Myokarditis, häufig auch als Perimyokarditis. Der histologische Nachweis der myokardialen Beteiligung hat eine nur geringe Sensitivität und imponiert als entzündliches Infiltrat mit Zelluntergängen in einer von mindestens 3 Biopsien. Der direkte Virusnachweis kann mithilfe der In-situ-Hybridisierung oder der PCR in Zellen aus pathologisch verändertem Gewebe geführt werden. Die viral bedingte Herzerkrankung ist in der Regel eine rein klinische Diagnose, die unter symptomatischer Behandlung meist folgenlos ausheilt. Bei chronischen Verläufen mit eingeschränkter linksventrikulärer Funktion und nachweislicher Viruspersistenz zeigten erste kleinere Studien vielversprechende Therapieansätze mit Immunglobulinen oder β-Interferon.
