Zusammenfassung
Nach den nicht zufriedenstellenden Ergebnissen der ersten Generationen von OSG-Endoprothesen, die vorwiegend als Zweikomponentenprothesen konstruiert waren, entwickelte Buechel eine Drei-Komponenten-Prothese (New Jersey LCS-Prothese, Depuy Warsaw, Indiana; Buechel et al. 1988; Buechel 1991; Buechel und Pappas 1992), die auf der Idee der meniskalen Kniegelenksendoprothese von Goodfellow (Goodfellow J und O’Connor J (1978); Giannini et al. 2000) basierte und deren Weiterentwicklung die Buechel-Pappas-Prothese (Endotech, South Orange, USA; Buechel et al. 2002) darstellt. Etwa gleichzeitig wurde ein zweiter Vertreter dieses Prothesentyps, die S.T.A.R.-Prothese (Scandinavian Total Ankle Replacement, Waldemar Link GmbH, Hamburg) von Kofoed entwickelt (Kofoed und Danborg 1995). Bei beiden bewegt sich ein Polyethylen-Gleitkern als dritte Komponente zwischen zwei polierten, einerseits in der Tibiaunterfläche, andererseits auf der Talusrolle fixierten Metallkomponenten, wodurch zwei getrennte Gelenkkompartimente entstehen (Buechel et al. 2003). Bei beiden Modellen ist die tibiale gelenkbildende Seite der Metallkomponente flach, während die gelenkbildende Seite der Taluskomponente durch ihre Formgebung und durch die korrespondierende Formung der Unterfläche des PE-Gleitkerns diesen führt und dessen Luxation verhindern soll (Easley et al. 2002). Durch die freie Verschieblichkeit des Gleitkerns gegenüber der Tibiakomponente werden die Friktion zwischen dem Polyethylen und der tibialen als auch talaren Komponente deutlich vermindert (Easley et al. 2002; Wood et al. 2000), wodurch die Scherkräfte auf die tibiale und talare Metallkomponente und die Implantat-Knochen-Grenzfläche minimiert werden (Easley et al. 2002; Wood et al. 2000). Unterstützt wird dies durch die hohe Konformität der miteinander artikulierenden Komponenten. Hierdurch wurde die zementfreie Fixation der Prothesenkompontenten möglich (Easley et al. 2002; Fink et al. 1999; Wood et al. 2000). Die etwas unterschiedliche Ausformung der talaren Gelenkfläche führt bei der S.T.A.R.-Prothese dazu, dass eine geringe Eversion-Inversions-Bewegung möglich ist, die hierdurch Randbelastungen reduzieren hilft (Buechel et al. 2003; Easley et al. 2002; Saltzman 2000).
