Abstract

Fractures of the C2 odontoid process are the most frequent injuries of the upper cervical spine (UCS). The majority of them are located at the base of the process (Type II). Less frequently, the fracture line passes through the C2 body (Type III) and exceptionally it remains above the transverse ligament (Type I). Contemporary treatment of choice in type II and shallow type III fractures is direct compressive osteosynthesis with one or two screws. However, other treatment modalities can be used to treat certain types of Type II fractures. Odontoid tip abruptions and broadbased odontoid-C2 body fractures are, usually, successfully treated with external bracing.
