Zusammenfassung
Ein 82-jähriger Patient mit COPD-Anamnese wird abends mit erheblicher Luftnot von der Normalstation auf die Intensivstation übernommen, nachdem bei ihm am Vormittag links infraklavikulär ein Schrittmacher implantiert worden war. Die pulsoxymetrisch gemessene Sauerstoffsättigung beträgt 78%, bessert sich aber unter 8 l/min Sauerstoffgabe über eine Gesichtsmaske auf 96%. Der Patient ist kurzatmig und die Thoraxauskultation dadurch erschwert. Das Atemgeräusch ist links und rechts leise, vielleicht linkslateral etwas abgeschwächt. Während der Intensivarzt eine Venenverweilkanüle anlegt, überlegt er das weitere Vorgehen.
