Abstract

Lung wedge resection by open surgery is performed to remove a portion of lung parenchyma usually smaller than a segment for therapeutic or diagnostic purposes, in patients in whom the resection is difficult to accomplish by video-assisted thoracoscopic surgery (VATS). This approach is used, for example, when the target area is on the diaphragmatic surface of the lower lobes or on the mediastinal surface close to the hilum of the upper lobes. Ground-glass lesions, most of which are non- or minimally invasive adenocarcinomas, are often difficult to palpate and their margins difficult to identify. Extensive dissection of pleural adhesions may also be too difficult and time-consuming to accomplish by VATS. Indications for open wedge resection are identical to those of VATS wedge resection, including management of pneumothorax and advanced emphysema, diagnosis of interstitial lung diseases and solitary pulmonary nodules, and surgical management of pulmonary metastases and early-stage lung cancers.
