Leitlinien zu Diagnostik und Therapie in der Gefäßchirurgie

Zusammenfassung

Als arteriovenöse (AV-) Gefäßzugänge zur chronischen Hämodialysebehandlung kommen native Fisteln und Prothesenshunts zum Einsatz. Wegen ihrer deutlich besseren Funktionsraten stellt die AV-Fistel den Gefäßzugang der ersten Wahl dar. So liegt die primäre 1-bis 2-Jahres-Offenheitsrate von AV-Fisteln bei 85–90%, diejenigen von Prothesenshunts lediglich bei 40–60% [12]. Die folgenden Leitlinien beschäftigen sich mit der operativen Anlage von AV-Gefäßzugängen sowie der chirurgischen und radiologisch-interventionellen Therapie von Komplikationen. Sie beruhen auf zwei aktuellen europäischen Leitlinien [1, 42], einer Literaturrecherche der letzten 20 Jahre sowie den eigenen Erfahrungen des Autors und einem nachfolgenden Prozess zur Konsensusbildung.
