Zusammenfassung

Ähnlich wie auf anderen Gebieten der Zytologie hat sich während der letzten anderthalb Jahrzehnte die Diagnostik der Brustdrüsenerkrankungen gewandelt. Noch vor wenigen Jahren war die Mamma das am häufigsten mittels FNA untersuchte Organ. Heute werden vor allem Mamillensekrete und Zystenpunktate, auch Frischgewebsabstriche von Sentinellymphknoten [21] und resezierten Mammakarzinomen zytologisch untersucht, während vielerorts die Stanzbiopsie die FNA verdrängt hat. Die Gründe dieser Entwicklung sind vielfältig. Ein wichtiger Grund ist der Mangel an erfahrenen Zytopathologen. Es ist anzunehmen, dass durch den Rückgang der FNA der Mamma auch an etablierten zytologischen Zentren wichtige Erfahrung verlorengeht und das, obwohl keineswegs feststeht, dass dieser Trend tatsächlich anhält. Denn es ist damit zu rechnen, dass die Indikationen zu molekularbiologischen Untersuchungen z. B. auf Her2-Überexpression, die am zuverlässigsten an zytologischen Präparaten gelingen, in den nächsten Jahren zunehmen. Außerdem wird die FNA ihre Bedeutung in der Metastasendiagnostik behalten. Daher erscheint es uns nach wie vor notwendig, sich ausführlich mit den Möglichkeiten der Mammazytologie zu beschäftigen.
