Zusammenfassung
Durchblutungsfördernde Mittel haben ihre einst führende Position bei der Behandlung peripherer arterieller Durchblutungsstörungen verloren. Seit 1992 ist das Verordnungsvolumen um 82% zurückgegangen. Damit wurde die praktische Arzneitherapie kontinuierlich an die neuen therapeutischen Möglichkeiten angepasst. Die Mehrzahl der Präparate ist zwar nach dem Arzneimittelgesetz zugelassen worden, hat aber nur einen fraglichen klinischen Nutzen für die Behandlung der peripheren arteriellen Verschlusskrankheit und der Claudicatio intermittens (Hiatt 2002). Wesentliche Komponenten einer optimalen Behandlung der peripheren Verschlusskrankheit sind die Senkung von Risikofaktoren durch Aufgabe des Rauchens, Senkung erhöhter Cholesterinwerte, Behandlung eines bestehenden Diabetes mellitus und Normalisierung eines erhöhten Blutdrucks (Hiatt 2001, Arzneimittelkommission der deutschen Ärzteschaft 2004, Hirsch et al. 2006, Arain und Cooper 2008). Wichtigste nichtmedikamentöse Maßnahme ist ein systematisches Gehtraining. So ist in einer Cochrane-Übersicht gezeigt worden, dass durch eine Übungsbehandlung bei den meisten Patienten mit Claudicatio ein klinisch bedeutsamer Anstieg der Gehstrecke (150%) erreicht werden kann (Leng et al. 2000). Darüber hinaus ist die Arzneitherapie mit Thrombozytenaggregationshemmern und ACE-Hemmern in Betracht zu ziehen.
