Zusammenfassung
Pharynx und Mundhöhle sind der Inspektion/Endoskopie unmittelbar zugänglich, sodass Erkrankungen durch klinische Methoden aufgedeckt werden können. Karzinome sind die weitaus häufigste Indikation zur Bildgebung in diesem Bereich. Dabei besteht die Aufgabe darin, die klinisch nicht ausreichend beurteilbare Tiefenausdehnung, die Beteiligung zervikaler Kompartimente sowie den Bezug zu Knochen, Knorpel, Gefäßen und Nerven einzuschätzen. Ein perineurales Tumorwachstum kann nur zum Teil durch neurologische Ausfälle bzw. eine Schmerzsymptomatik vermutet werden. Vor der Therapie muss darüber jedoch Klarheit herrschen, da neben der histologischen Absicherung eine Tumorausdehnung entlang von Gefäßen und Nerven bzw. eine Zweitabsiedlung entfernt vom Primum einen grundlegenden Einfluss auf die Strategie — Operation vs. Radio-(Chemo-)Therapie — hat. Sowohl die lokale Tumorausdehnung als auch der Lymphknotenstatus müssen zur Festlegung der klinischen T- und N-Kategorie bildgebend eingeschätzt werden.
