Die Indikationen für eine TMS-Untersuchung der Hand — und Armmuskeln sind in der Übersicht in Abschn. 7.1 aufgelistet und gelten auch für die TMS-Diagnostik der Beinmuskulatur. Die Untersuchung der MEP der Hand- und Armmuskeln hilft in erster Linie bei der Beurteilung der Integrität kortikospinaler und peripherer motorischer Projektionen zu den oberen Extremitäten bei Patienten mit einer Kraftminderung der Arme. Es ist möglich, in klinisch nicht eindeutigen Situationen, bei Patienten mit Paresen den Läsionsort im kortikomotorischen System näher einzugrenzen, z. B. eine zentrale Läsion mit gestörter kortikospinaler Überleitung von einer peripheren Läsion abzugrenzen. Die Auswertung der ZML oder der morphologischen Analyse der von der Hand- und Armmuskeln abgeleiteten MEP erlaubt jedoch meist keine eindeutige Abgrenzung von kortikalen, subkortikalen oder spinalen Läsionsorten. Da nur ein kleiner Teil der absteigenden motorischen Bahnen der Untersuchung mit der TMS zugänglich ist (Kap. 3), schließen Normalbefunde einen pathologischen Prozess nicht aus. Die Sensitivität der TMS lässt sich durch Verwendung der Triple-Stimulationstechnik (TST) steigern (Kap. 11).
