TMS wird seit weit über 15 Jahren in der Neurologie zur Routinediagnostik der Leitfähigkeit motorischer Bahnen eingesetzt. Bei Schlaganfallpatienten hat sie vor allem prognostischen Wert in der Abschätzung noch vorhandener kortikospinaler Bahnen (Kap. 24). Die therapeutischen Möglichkeiten der TMS hingegen werden zwar oft als “erfolgsversprechend„ gewertet aufgrund ihres Potenzials, Veränderungen der kortikalen Erregbarkeit und LTP/LTD-ähnliche Prozesse zu induzieren. Einen Eingang in die klinische Praxis haben therapeutische Applikationen bei Schlaganfallpatienten bisher jedoch nicht gefunden (Kobayashi u. Pascual-Leone 2003).
