Im klinischen Alltag stützt sich die Diagnostik der Myelopathien gegenwärtig weitestgehend auf bildgebende Untersuchungen und hierbei vor allem auf das MRT. Gleichwohl haben sich die motorisch evozierten Potenziale (MEP) nicht nur als sehr sensitiv, sondern oft als sinnhafte komplementäre Untersuchung erwiesen. Bei der kompressiven zervikalen Myelopathie liegt die Sensitivität der MEP mit klinischer oder bildmorphologischer Referenz (MRT) bei annähernd 100%. Abhängig vom Referenz-Standard findet sich hinsichtlich Spezifität der MEP eine ungleich heterogenere Datenlage. Jüngere, prospektive Studien mit MRT-Referenz-Untersuchungen geben die Spezifität mit bis zu 98% an (Lo et al. 2006). Im Vergleich zu den somatosensorisch evozierten Potenzialen (SEP) sind die MEP bei der Diagnostik der Myelopathien insbesondere in frühen Stadien der Erkrankung sensitiver (Chistyakov et al. 2004). Zudem darf als gesichert gelten, dass mittels TMS auch klinisch stumme Läsionen in substanziellem Ausmaß (bis zu 65% der Fälle) detektiert werden können (Travlos et al. 1992). Hierbei ist festzuhalten, dass sich die Sensitivität mit zunehmender Anzahl der Ableitungen und Einbeziehung der Beinmuskeln (M. tibialis anterior) erhöht (Lo et al. 2006; Tavy et al. 1994).
