Abstract

In cardiac failure, arterial underfilling leads to neurohumeral activation and renal sodium and water retention, thus increasing both the preload and afterload, potentially setting up a vicious cycle leading to a cardiorenal syndrome. During acute decompensated heart failure, a deterioration in renal function is associated with a markedly increased risk of patient mortality.
