Das Mediastinum beinhaltet sehr unterschiedliche, zum Teil sehr relevante Strukturen und Organe. Es wird mit diesem Begriff der Raum zwischen den beiden Pleurasäcken bezeichnet, der von vorne vom Sternum und von hinten von der Brustwirbelsäule begrenzt wird und andererseits unten vom Zwerchfell und oben von der oberen Thoraxapertur umschlossen wird. Nach oben ist das Mediastinum nicht klar abgegrenzt, hier anschließend findet sich das lockere Bindegewebe des Halses. Um die verschiedenen Strukturen besser gruppieren zu können, ist es hilfreich, das Mediastinum in verschiedene Kompartimente einzuteilen (⊡ Abb. 13.1). Gebräuchlich sind eine Einteilung in 3 ( vorderes, mittleres und hinteres Mediastinum) oder 4 (zusätzlich oberes Mediastinum) Kompartimente. Diese Einteilung ist vor allem für die Neoplasien relevant (s. übersicht in ▸ Abschn. 10.4.2), hilft aber auch für die Zuordnung der anderen Veränderungen.
