Nichtsteroidale Antirheumatika (NSAR) sind die am häufigsten eingesetzte Medikamentengruppe. Sie hemmen die Cyclooxygenase (COX-1 und COX-2) und greifen damit in die Prostaglandinsynthese ein. Vor allem die analgetischen und antiphlogistischen Eigenschaften begründen die häufige Verwendung. Neben den rheumatischen Erkrankungen, für die im Wesentlichen NSAR rezeptiert werden, zählen Schmerzzustände, insbesondere Spannungskopfschmerzen, Fieber, Dysmenorrhoe, Sportverletzungen, aber auch der prophylaktische Einsatz für thromboembolische Ereignisse und intestinale Polyposis gehört zu den Indikationsgebieten. In vielen Ländern, so auch in Deutschland, sind NSAR z. T. rezeptfrei erhältlich, so dass die Selbstmedikation ohne direkte ärztliche Supervision eine wesentliche Rolle bei der Vermarktung spielt. In den USA werden gezielt verbraucherbezogene Werbekampagnen für NSAR durchgeführt. Mehr als 20% der Menschen über 65 Jahren nehmen häufig NSAR ein.
