Zusammenfassung

Magenulzera entstehen durch ein Ungleichgewicht zwischen aggressiven und defensiven Schutzmechanismen im Magen. Ein Großteil der Magenulzera ist durch die Infektion mit H. pylori bedingt und wird durch eine sog. Triple-Therapie behandelt. Zu den aggressiven Faktoren, die die Schleimhaut schädigen, zählt weiterhin die Exposition mit Gallensäuren. Zunehmend häufiger werden NSAR-Gastropathien beobachtet. NSAR schädigen die defensiven Mechanismen der Schleimhaut, vor allem durch die Ausschaltung des COX-1-Isoenzyms, das die Prostaglandinsekretion steuert. Patienten, die unter längerfristiger Medikation mit NSAR Schmerzen entwickeln, sollten mit H2-RA oder PPI prophylaktisch behandelt werden. NSAR-induzierte Läsionen neigen zu Blutungen, die meist endoskopisch gestillt werden können und unter hochdosierter Therapie mit antisekretorischen Medikamenten beschleunigt abheilen.
