Abstract

The anterior cervical discectomy and fusion (ACDF) is currently a favored treatment option for most degenerative cervical spine pathology. The ACDF has been shown to be a safe and effective treatment alternative for patients with cervical degenerative myelopathy and radiculopathy. The goals of anterior cervical spine surgery include stabilization of the cervical spine, maintenance of cervical alignment, and decompression of the neural structures, all of which have improved with the use of an anterior cervical plate, especially in smokers. This chapter addresses the fundamental biomechanics and development of the anterior cervical plate, the surgical nuances of securing the anterior cervical plate, and the radiographic and clinical outcomes of anterior cervical plating in degenerative myelopathy and radiculopathy.
