Abstract
Peptic ulcer disease occupies a prominent position in the development of modern general surgery and surgical thought. Surgical management has progressed from emptying procedures to patching procedures to physiologic operations involving nerve division, as the physiology of gastric function and acid production was better understood. The introduction of effective medical therapy to suppress acid production has significantly reduced the incidence of peptic ulcer disease requiring surgery, while the discovery of Helicobacter pylori gave an explanatory cause. Despite therapeutic targets, patients with complications of peptic ulcer disease requiring surgical therapy still arrive on the doorsteps of emergency departments across the world. Peptic ulcer disease remains an important part of general surgery training and practice.
