Gonadal Dysfunction in Males with Overweight or Obesity, and Adiposopathy

Abstract

Although obesity has become endemic, the reproductive problems in men with obesity who do not have diabetes mellitus (DM) or dysmetabolic syndrome, have not been studied well. This chapter summarizes the information available to date, in order to provide the clinician with suggestions on how to approach the management of these patients. This chapter introduces the concepts of bariatric hypogonadism, hypogonadal overweight or obesity, and bariatric erectile dysfunction (ED). The role of adiposopathy in these problems is addressed, the complexity of the evaluation is summarized, and the available management options are presented.

The relationship between obesity and hypogonadism in men is complex and bidirectional, creating a clinical “perfect storm” due to a pathological vicious cycle. However, in spite of the paucity in available information, effective management options are available to help these patients.
