Patch stage:

Ill-defined dermal lesion

Mild increase of elongated vessels with minimal endothelial atypia

Perivascular lymphocytes and plasma cells
-
Vessels dissect between collagen and surround larger vessels (the “promontory” sign)
Plaques stage:
-
Easily detected dermal vascular proliferation
-
Spindle cells with “slit-like” vascular spaces (HHV8+)
-
Red blood cell extravasation with some hemosiderin
Nodular stage:
-
Nodular proliferation of atypical spindle cells with delicate “slit-like” vascular spaces
-
Red blood extravasation and hemosiderin can be seen
