Abstract

It is imperative for clinicians to have a good documentation of the assessment and treatment interventions conducted with each patient. The failure to document suicide risk assessments and interventions may give the court reason to conclude they were not done when malpractice complaints arise.

This chapter lists the documentation requirement for inpatient units when patients are admitted for suicide-related issues and in outpatient settings when patients are seen in the community.
