Abstract
Creating a healthcare transition program for a practice, institution, or region requires engagement of pediatric providers serving in a variety of roles. Guidelines for best practices from pediatric providers have been set but require high level planning, creation of tools, and motivation to change workflows. Using two case examples, we present a conceptual summary of these roles, structural supports, and motivational tools, with the ultimate goal of incorporating transition best practices into routine pediatric practice.
