Organizational Innovation in Surgical Critical Care
Critical care is an organization tasked with the most complex mission in healthcare: to concentrate, project, and distribute the finite resource of highly specialized medical attention to a population of vulnerable patients in an effective, efficient, and reliable way. Often applied at the extremes of age and life, critical care is an arbiter of patient safety and outcome salvage, and its performance is increasingly used as a measure of an institution’s overall quality. A critical care organization should therefore be the result of deliberate, collaborative decisions informed by a comprehensive understanding of the entire care system rather than the piecemeal accretion of individual responses to isolated events. This chapter provides means and rationales for models of critical care accessibility, responsibility, and accountability as well as recommendations for developing a performance-based organizational culture.
