Abstract

An ambulatory facility of a hemopoietic stem cell transplant (HSCT) service is essential for the purposes of pre- and post-transplant care of the patients and donors in the setting of allogeneic transplantation. The care of patients in this facility will be optimized by the presence of well-established protocols for the usual procedures undertaken there, which may include medical evaluation, fluid and electrolyte replacement, delivery of specialized parenteral medications, and in the case of some facilities, the delivery of conditioning chemotherapy and the infusion of the stem cell product itself. Fundamental to a successful ambulatory facility is an adequately trained and staffed nursing contingent; access to pharmacy, blood bank, and investigative services; and close relations with an emergency care facility, as well as inpatient wards capable of accepting acutely and sub-acutely ill patients after HSCT. Ideally, the medical staff will be part of the same team caring for the patients when they are inpatients. An efficiently run ambulatory facility for HSCT patients will have a consulting room facility nearby or integrated within the ambulatory care center.
