Abstract

There is a dearth of literature specific to geriatric trauma patients. Although there are nutrition risk assessment tools that have been validated in the elderly and even elderly ICU patients, few multisystem trauma patients are included in these analyses. The American Society of Parenteral and Enteral Nutrition (ASPEN) and the Society of Critical Care Medicine (SCCM) have recently updated their guidelines for the provision of nutrition therapy in adult critically ill patients, including recommendations for trauma subsets such as traumatic brain injury and burns. Although a practitioner can use these guidelines as a starting point when designing a nutrition therapy plan for the elderly trauma patient, attention must also be paid to pre-existing comorbidities as well as the likely presence of declining organ function, nutrient deficiencies, and fluid abnormalities that are common in the elderly patient. This chapter provides a review of common metabolic and physiologic changes in the elderly, tools for performing nutrition risk assessment, and options for the type, route, and timing of nutrition support in geriatric trauma patients.
