Cardiac Surveillance Whilst Listed for Renal Transplantation

Abstract

The renal transplant waiting list in the United States has almost doubled since 2002 [1]. Annual mortality whilst listed is estimated at 5% [2], a third of cases with a documented cause are attributable to cardiovascular disease. In this chapter, we discuss the role, if any, of periodic cardiac surveillance following the initial cardiac evaluation and listing. In this context, the aim of performing routine periodic screening is to sensitively and specifically predict which patients are likely to experience cardiac events while awaiting transplantation, and to correct these as to minimize post-transplant complications. The discussion will be based on recommendations from existing guidelines and relevant published literature, specifically relating to coronary artery disease, valvular heart disease, and left ventricular dysfunction. Based on existing data, there is a paucity of published studies to provide definitive advice as to the utility of surveillance screening for those with a normal initial assessment who are listed for renal transplantation. Prospective research to assess the prognostic value of interval surveillance whilst awaiting transplant is required.
