Interventions in Structural Heart Diseases: Tricuspid Valve Regurgitation

Tricuspid valve disease is mainly represented by tricuspid regurgitation (TR), which is a predictor of poor outcome. Functional (or secondary) TR is the most frequent tricuspid valve disease and correlates with an increased risk of cardiovascular events and death in long-term follow-up. Tricuspid surgery for severe TR should be preferably performed during left valve surgery. When TR persists or worsens following left valvular surgery, redo isolated tricuspid surgery is associated with high mortality. Recently, interest in pathophysiology and management of TR has been growing and new treatment options have been developed. In patients with previous tricuspid surgery, transcatheter valve-in-valve or valve-in-ring procedures can be performed using a transfemoral or transjugular approach. In patients with native disease, many percutaneous techniques mimicking surgical techniques are coming to light and novel transcatheter therapies for the treatment of TR are now emerging. Current experience with these new therapies is preliminary and further studies are needed to determine their role in the management of this clinically relevant disorder.
