Abstract

Around 150 patients per 100,000 inhabitants in the western world live with a stoma for various reasons. Half of these patients will have a reversal of their stoma. In patients having a stoma for lifetime, the quality of life can be reduced by having the stoma per se, or get even worse, when having an associate complication like a parastomal hernia.

A parastomal hernia is commonly seen in stoma patients varying in frequency due to type of stoma and the position of it in the abdominal wall. Operative factors and the patient’s own risk factors can affect the outcome. The techniques lounged for the repair of a parastomal hernia are several and the results are not too encouraging with several associated problems. It can though be concluded that a simple suture repair will not do. The development of techniques used for mesh placement (open and laparoscopic), various positions of the mesh in the abdominal wall as well as the types of meshes (material, coatings, fixation and specially designed) are factors that are difficult to study for outcome in this huge mass of factors that could influence the outcome.

Prophylactic mesh reinforcement has been launched with the aim to reduce the risk for the development of a parastomal hernia. The results seem favourable, but the optimal indications and results for specific stoma types are still under debate.

There is still a lot of work to be done for the stoma patients. The golden opportunity will be when crating the stoma.
