Management of the Most Difficult Perioperative and Technical Challenges in Abdominal Transplantation

Abstract

Transplant surgery is one of the most challenging surgical subspecialties and requires broad training in a wide array of general, vascular, gastrointestinal, hepato-pancreatico-biliary, and pediatric surgery as well as urology procedures. Despite the surgeons’ comprehensive training, technically extremely difficult situations can arise during transplant procedures that not only require technical mastery but also a high degree of flexibility, creativity, swiftness, and experience. Some of these situations are unexpected challenges, which may include unforeseen conditions that were not present or recognized on previous imaging studies, sudden intraoperative complications or anatomical variants that prevent the execution of a standard procedure. This chapter focuses on the diagnosis and treatment of these types of unusual or unforeseen conditions and technical challenges in abdominal transplantation, which may occur intraoperatively but also immediately pre- or posttransplant. Knowledge of these complications is crucial in preventing poor graft and patient outcome.
